Freedom of Information requests

You can request access to certain records under the Victorian Freedom of Information Act. 

Applications must be made in writing. A $24.40 application fee applies.

 

How to make a Freedom of Information request

Download and complete the application form, or write to us detailing your request.

Send the form with a cheque or money order for $24.40 to:

Please provide enough details for us to find the right documents. Avoid phrases like "all documents about" a topic. If your request is not clear, we will ask you for clarification which may delay your request.

If you ask for documents about your personal affairs, we may ask you to provide proof of your identity. If you ask for documents on behalf of someone else we may ask you for proof of their consent.

How we respond

We may release all, part or none of the documents you request. We will give you a decision about your request no later than 45 days after it is received or clarified.

If we decide not to release documents, we will give you reasons and tell you how to appeal.

 

Fees and charges

There is a fixed, non-refundable $24.40 application fee for all Victorian Freedom of Information requests.

Please send a cheque or money order with your request.

If payment would cause you hardship you can ask for it to be waived.
